Physical Exams for Spring Season Athletes
Feb. 17, 2017 -- Students in grades 7-12 who will play on a spring season athletic team must have an annual physical exam before participating.
The physical exam needs to be completed once during the school year.
The Athletic Department will host a physical exam evening with a local physician conducting the exams on Thursday, February 23, at 6:30 p.m. in conference room 120, across from the Media Center.
Parents – please complete the parent portions of the Pre-Participation Physical Examination Form before coming to the exam with your student athlete.
The doctor will complete the remaining portion of the form.
The cost of the physical exam is $10, cash only.
All Ohio student-athletes are required by the Ohio High School Athletic Association to have an annual physical exam before participating in athletics.
