We thank you for choosing Heights and entrusting the CH-UH City School District with the education of your children.

If you are interested in registering your child for preschool or pre-kindergarten, please see our Early Childhood Program page or call 216-371-7356 for more information.

Before you begin the enrollment process, please enter your address into our interactive boundary map to confirm that your residence is within the District boundaries. The map will also show you which school your child will attend. Please call the Registration Department at (216) 371-7430 with any questions.

K-12 Online Registration

Step 1 - Complete the Online Registration. (Have documents listed below ready to upload onto the application)

Step 2 - Scan and upload the required documents directly to the application, or take pictures of the documents and email them to chuhreg@chuh.org. You may also fax to 216-371-7177.

The Online Registration portal allows you to save your progress and upload documents as you complete the application. However, the Registration Office will only begin processing your registration once it has been fully completed and submitted.

For families bringing documents in person, the Registration Office only processes complete registrations and does not accept or maintain partial documents.

Required Documents:

Birth Certificate - A copy of the child's birth certificate, passport, or equivalent
Notarized Residency and Custody Affidavit - Required for all families; one per household
Parent/Guardian Identification - Ohio driver's license or photo identification card (ex. passport, County worker ID)
Record of all Required Immunizations (find exemption form here)
Ohio Medicaid Consent Form (provides one-time consent for the reimbursement of Medicaid-qualified services provided by the district)
Proofs of Residency in school district (2 items)
- Mail - ONE piece of business mail (such as utility bill, bank statement, pay stub, current vehicle registration, voter registration, other government documentation) addressed to parent/legal guardian dated within the last 60 days (water/sewer bills are not accepted)
- Residency Status - Provide 1 of the following in addition to the mail requirement above:
  - Homeowner – mortgage document or purchase agreement with a closing date
  - Renter – current signed lease. Leases must include the leasing agent or landlord’s current contact information.
    - If Parent/Guardian is not the homeowner or leaseholder, the Parent/Guardian must complete the Shared Residence Affidavit AND the homeowner or leaseholder of the home must provide one piece of business mail and either the mortgage/purchase agreement or current signed lease.
Consent for Release of Records (only necessary if your child previously attended a different K-12 school.)
Asthma Action Plan (only necessary if your child has asthma.)

Custody Papers, if applicable - Court documents (certified copy) required, as follows:

Divorce - Divorce decree (must establish residential parent for schooling purposes)
Custody - Journal entry from Juvenile Court
Guardianship - Letter of Guardianship from Probate Court

School Records, if applicable

Withdrawal form and current report card from previous school
Individualized Education Plan (IEP), Evaluation Team Report (ETR), and/or 504 Plan if the student received Special Education services at their previous school

Who may register a child?
Only an adult who resides within the Cleveland Heights-University Heights City School District and who is the parent or legal guardian of the child may complete the registration process. All certified court orders pertaining to guardianship and custody must be presented at the time of registration.

Residency Requirements
The Cleveland Heights-University Heights City School District is tuition-free only for those students whose parents/legal guardians are residents of the school district. Residence for school purposes means that the parents/legal guardians of the student live within the school district seven days a week.

A person can only have one “residence” for school purposes at a time, and residency is not linked to the property the parent/guardian may own. A person's legal residence is where substantial family activity takes place (where the parent/guardian eats meals, sleeps, and receives mail).

After Enrollment

Parents/guardians will receive information from their child's school (e.g. open houses, orientations, supply lists) at the start of the school year. Our Back to School page contains this information as well.

We strongly recommend completing the registration process for your child no later than July 31. Any registration received after July 31 may result in your child not being able to start on the first day of school.

Families who do not provide all of the required documentation at the time of registration may experience delays in enrollment. Incomplete submissions may also be subject to further review and verification. We thank you for your patience and understanding.

2026-2027 Intradistrict Transfer Process

Applications for Intradistrict Transfers are available through the Infinite Campus Parent Portal as an online registration form. Parents/guardians must sign in to the Parent Portal to complete and submit the application.

The District will accept Intradistrict Transfer applications for the 2026-2027 school year from March 9, 2026 through April 17, 2026.

Parents/guardians will be notified of the decision regarding their application by mail no later than May 8, 2026.

Please note that all applications submitted during the application window will be reviewed using the District’s stated criteria. Applications are not processed on a first-come, first-served basis.

Families with students entering Kindergarten for the 2026-2027 school year who wish to request a school transfer through the Intradistrict Transfer process must:

Submit an Intradistrict Transfer application, and
Complete the student registration process during the application window.

Application Review Process

Elementary and Middle School Requests

The District will only consider Intradistrict Transfer requests that meet the following criteria:

The student must be a current resident of the Cleveland Heights–University Heights School District.
The student will be enrolled in Kindergarten through 8th grade for the 2026–2027 school year.
The projected enrollment and available space at the requested school building allow for consideration of transfer requests.

Once the District determines that space is available at the elementary or middle school level, applications will be processed as follows:

Sibling Priority: Preference will first be given to students who have a sibling currently attending the requested school.
Randomized Selection: After sibling priority is applied, any remaining available seats will be filled using a randomized selection process.

Instructions for Withdrawals

Withdrawals are completed at the school your child attends. You will need the following information: Photo ID and the reason for the withdrawal. If you are moving out of the CH-UH District and/or state of Ohio, the new address and school information must be provided. If you have questions, please contact the appropriate school.

NOTE: Prior to withdrawing, all school-issued items are returned to the school (e.g. library books and musical instruments) and all district-issued Chromebooks must be returned to IT at the Delisle Options Center (14780 Superior Rd Door #7).

Kindergarten Registration For 4-Year-Olds/HB 114

The State of Ohio has recently mandated changes to Kindergarten enrollment requirements, per House Bill 114. The District has developed policies and processes that will allow us to continue our practice of enrolling students who turn five by September 30. These policies and processes were created in partnership with our kindergarten teachers, District and school leadership, and legal counsel to ensure they adhere to the law and apply sound educational practices. They were approved by the CH-UH Board of Education on March 11, 2026.

If your child turns five after August 19, 2026 but on or before September 30, 2026, you may submit their registration for kindergarten using our Online Registration Portal (find the portal link and required documents in the "K-12 Online Registration" tab above). Once your registration is complete and submitted, it will initially be classified as pending and not finalized. You will receive notification from the District regarding the next steps, listed here:

You will be invited to bring your child to a kindergarten readiness screening, which typically takes about an hour. The readiness screening will be administered by a District staff member who possesses expertise in early childhood development. Your registration must be complete and submitted in order to receive this invitation for a screening.
The District will use this screening to determine whether to admit your child to kindergarten. The results of the screening, along with the District’s admission decision, will be shared with you. Your child can then be enrolled in kindergarten if both you and the District agree that your child is ready.

Registration for incoming kindergartners who are 5 years old by (or on) August 19 will not be impacted by HB 114 and will proceed as usual.

Early Entrance to Kindergarten

Please note that this process is separate from the Early Entrance to Kindergarten program that applies to children who turn 5 after September 30 and before January 1. An Early Entrance evaluation is available for children with birth dates from October 1 to December 31 of the year they will attend school, and who also demonstrate superior functioning in the areas of achievement or ability.
