Families never pay out-of-pocket costs for their student's care through this service.
MetroHealth will bill insurance when able. If you do not have insurance, our team can help connect you to insurance resources.

- Injury and Illness Care
- Routine Lab Tests
- Prescription Medications
- Teen Services
- Care for Chronic Conditions (asthma, allergies, diabetes, & seizure disorders)
- Mental/Behavioral Health
- Immunizations
- Dental (screenings, cleanings, fluoride, sealants)
How to enroll your student
Fill out an online consent form here. Students' consent forms remain valid for as long as they attend CH-UH City School District schools. You do not have to switch providers or hospital systems to use the MetroHealth School Health Program services.
How to schedule an appointment
Call the MetroHealth School Health Program at 216-957-1303 or email SHPschedule@metrohealth.org. You can also request an appointment through your school nurse.
CH-UH City School District families and employees can also receive routine health care at the Heights Wellness Center. Make an appointment by calling 216-957-1303 or emailing SHPschedule@metrohealth.org.
