Pursuant to Ohio Rev. Code 3313.473(B)(4), we write to inform you of all health care services available to our students through the School District and in connection with outside providers. As a parent, you may withhold consent or decline care on behalf of your child for any of the physical, mental, or behavioral health care services listed below.
