Apr. 22, 2021 -- On Wednesday, April 28, Heights High School will host a COVID-19 vaccine clinic at the school in partnership with Giant Eagle Pharmacy and the Educational Service Center. Students, family members, employees and community members 16 and older will have the opportunity to receive dose #1 of the Pfizer vaccine.
Please fill out this short Google Form by Friday, April 23 if you, your student, or a family member would like to receive the vaccine at this clinic. If you have already registered your student, you do not need to register them again. The District and the ESC will use this information to schedule appointments, as well as for staffing and logistical purposes.
The clinic on April 28 will begin at 8 a.m. at Heights High School. The end time will be determined by the number of sign-ups received.
There is no cost to receive the vaccine.
The provider is Giant Eagle Pharmacy (see attached letter).
A CH-UH school nurse will be present at the clinic.
School leaders will work to schedule students’ appointments around OST testing, if applicable.
Everyone will need to complete and bring the attached “Schools COVID Vaccination Form” to the clinic. It must be signed by a parent/guardian if the recipient is under 18. Print double-sided on 1 sheet of paper.
Vaccine recipients should bring the following with them:
Proof of date of birth (e.g. driver’s license, state issued ID, birth certificate)
Medical insurance (and prescription insurance, if separate). If the person receiving the vaccine does not have insurance, they must include their full Social Security number on the vaccination form.
Participants must wear a mask, and should wear short sleeves if possible.
The second dose clinic will take place on Wednesday, May 19. You must be able to attend both dates.
