May 17, 2021 -- Back in July, District counselors and social workers asked PreK-12 students to complete a needs assessment survey ahead of the new school year. Now, they would like for students to take another survey to gather data on how those needs may have changed.

The surveys ask students about their stress levels and concerns. The middle/high school level survey asks students if they have experienced the loss of a loved one during the pandemic and if their basic needs are being met. Parents/guardians may help students who need assistance completing the survey.
 
Survey for PreK-5 (preschool & elementary students)

CH-UH staff members are also asked to complete this survey to help the teams understand specific needs of students.
 
Students and staff are asked to complete their surveys by May 28, 2021.
