Social-Emotional & Basic Needs Surveys Available For PreK-12 Students
July 27, 2020 -- CH-UH City School District counselors and social workers are busy planning for the 2020-2021 school year. To help them best prepare, PreK-12 students are asked to complete a needs assessment survey.
 
The surveys ask students about their stress levels and concerns. The middle/high school level survey asks students if they have experienced the loss of a loved one during the pandemic and if their basic needs are being met. Parents/guardians may help students who need assistance completing the survey.
 
Survey for PreK-5 (preschool & elementary students)
Survey for 6-12 (middle & high school students)
 
CH-UH staff members are also asked to complete this survey to help the teams understand specific needs of students.
 
Students and staff are encouraged to complete their surveys by September 30 in order to give counselors and social workers time to prepare and plan ahead of the start of school.
