CH-UH, MetroHealth Hosting Student Immunization Clinics
Nov. 4, 2020 -- The District nursing team is working with MetroHealth School-Based Health Services to provide no-cost immunization clinics for any CH-UH students who need additional required immunizations for school attendance and/or the flu vaccine.

An appointment for the clinic is required. Contact Nurse Rose Varcelli at 216-320-3083 to schedule an appointment at any of the following locations:

Heights High School on November 11
Roxboro Middle School on November 12
Monticello Middle School on November 18

The clinics will run from 9 am to 5:30 pm at all three locations.

After scheduling an appointment, please fill out the MetroHealth registration form, which is a fillable PDF that you can complete online and submit to [email protected]. A copy will be available for your signature at the time of your student’s visit. If you have a paper copy, you may bring it along to your student's appointment or send clear photos of all pages by email to [email protected].  

Insurance will be billed for these services, but no co-pays or deductibles will be collected. If there is no insurance, your student will still be able to receive immunizations without cost. Completing the MetroHealth registration form also allows MetroHealth to provide care during future visits without additional registrations.  

Please have any immunization documentation available for the MetroHealth providers. Your student's current school immunization records will be provided to the MetroHealth provider.
