Work permits are required for students who are 14 -18 years of age and who have a job offer (students who are 16 and 17 ARE NOT REQUIRED
to have work permits for summer employment). During the COVID school closure, the Ohio Department of Commerce
has authorized school districts to use a completed application as a permit.Instructions for students who need a work permit:
1. Print Application for Minor Work Permit.
2. Complete the Student/Applicant section. The Superintendent’s signature is not required during the COVID school closure.
3. Contact your physician or the last physician who conducted a physical for the student and ask how to complete the physician's portion of the application.
4. Give the application to the employer. They will complete their portion and keep for their records.
If students need an application, and do not have access to a printer, they may pick up applications at Heights High Monday - Friday, from 10:00 a.m. - 1:30 p.m. at the meal distribution location.
During the COVID school closure, the application will serve as the permit.
When schools open, we will again produce the permit after receiving the application.
Each change of employment requires a new application.
