In order for your child to receive a free or reduced lunch during this school year, an application for 2015-2016 must be submitted by Thursday, October 15.
The applications are available on CHUH.org by clicking here, or can be picked up from your child’s school.
Please return the completed application to the cafeteria cashier, mail it to your child’s school, or fax it to 216-397-3618.
